GOING BENEATH THE SURFACE: WHAT PEOPLE WANT FROM THERAPY

In these last few years, we have witnessed unprecedented upheaval in the areas of politics, social justice, the natural world, and public health. To help our patients and to support our practices, communities and the field as a whole, it is vitally important to understand what people are looking for in mental health treatment, and how they think and feel about the therapies we offer. It’s also crucial that public awareness and appreciation of therapies of depth, insight, and relationship improve. These therapies have been missing from, if not pushed out of, the public consciousness, and are misunderstood, undervalued, and overlooked by the general public. To assess what people know about therapy and mental health and what they really want from treatment, the Psychotherapy Action Network (PsiAN) conducted extensive original research with the general public. We first conducted research in 2020, and now have new research to offer a post-pandemic perspective on the attitudes and expectations people have towards therapy, which benefits matter most, what their misperceptions and biases are, and what they know and don’t know about mental health and therapy. The presentation will also offer a blueprint for engaging and communicating with the public, so that the public gains greater awareness, understanding and appreciation for therapies of depth, insight, and relationship.
Learning Objectives:

1) describe the benefits that the general public seeks from therapy and their attitudes towards issues of mental health care.
2) describe the public’s stated obstacles to seeking mental healthcare.
3) be able to list four key elements that should be present when communicating with the general public about psychoanalytic psychotherapy and psychoanalysis.

This presentation delves into the complex nature of Narcissistic Personality Disorder (NPD), focusing on the defining trait of a grandiose sense of self. It explores how individuals with NPD develop an inflated self-image and exaggerated sense of abilities, leading to manipulative behaviors, lack of empathy, and shallow relationships. Drawing on the work of Bion, Steiner, De Masi, Rosenfeld, and many other contemporary Object Relations theorists, we explore the dynamics of this structure and its impact on individual identity and behavior. By examining the underlying psychological structures, the presentation aims to shed light on the seductive yet destructive nature of narcissistic grandiosity and its profound impact on identity and interactions.
Through a detailed case study of "Barry," and of several other patients, the presentation offers practical insights into the lived experience of maintaining a grandiose self. It highlights the psychological mechanisms that drive individuals to uphold illusions of superiority, despite underlying feelings of worthlessness and past traumas. Attendees will gain a deeper understanding of the challenges in treating NPD and the therapeutic approaches that can foster genuine change and emotional connection.

1) be able to identify key traits of narcissistic personality disorder focusing on the grandiose sense of self.
2) gain an understanding of the grandiose sense of self as a core feature of NPD, drawing on contemporary Object Relations theory, and learn how this structure develops, its impact on individual identity and behavior, and the mechanisms through which it maintains control over the personality.
3) learn practical therapeutic approaches for treating individuals with NPD, focusing on how to address and transform the grandiose self-structure, emphasizing strategies to dismantle delusional narratives and foster genuine self-awareness and growth.
